2007 ABIM Foundation Forum - Coordination of Care: Missed Opportunity
The 2007 ABIM Foundation Forum convened more than 120 health care leaders to identify successful models, organizational strategies, and structural and financial changes needed for the health care system to improve care coordination. From Forum to Function: Bringing Care Coordination Home summarizes the Forum’s goals and how participants put the Forum’s recommendations into action. An article based on Thomas Bodenheimer’s Kimball Lecture was also published in The New England Journal of Medicine.
Care Coordination Measures
The ABIM Foundation has sponsored a number of initiatives related to care coordination and transitions, including convening the Stepping Up to the Plate Alliance (SUTTP) and the 2007 Forum. This work has continued most recently through the Physician Consortium for Performance Improvement (PCPI)’s development of measures related to care transitions, and the next phase of the SUTTP, which is focused on building links between transitions for hospital and primary care.
